What to Know if You’re Delivering at SMH

Finding out you’re pregnant can be exciting and overwhelming. To make your labor and delivery go as smoothly as possible, complete the preadmission process, schedule a tour of our Slidell Memorial Hospital Birthing Center, and register for a prenatal class.

Complete the Preadmission Process

Filling out the necessary paperwork beforehand will ensure we have your medical information on file should complications arise.

There are several ways you can complete the preadmission process:

  1. Print and complete the online pre-admission registration form and bring it to your scheduled SMH Birthing Center tour.
  2. Bring the form to SMH Admitting, Monday through Friday, 6:30 a.m. to 5 p.m.
  3. Mail the form to:

  4. Call (985) 280-8558 to register by phone, Monday through Friday, 8 a.m. to 5 p.m.

During the preadmission process, you can choose how you want to pay for your delivery. If you have insurance, bring all information to your preadmission appointment. Slidell Memorial will file your insurance claims and collect your co-insurance. If you don’t have insurance, our financial counselor can help arrange an affordable financial plan. Birth Certificates

SMH’s Health Information Management Department assists parents by completing newborn birth certificate applications.  Please read and follow the directions in this form. Please complete the Mother's Worksheet 40 days before your delivery and email the completed copy to BCSlidellmain@ochsner.org. You can also complete the form and turn it in during the pre-administration process. The birth certificate clerk is available during the following hours to assist you.

  • Monday ­– Friday: 7:30 a.m. to 4 p.m.

Birth Registration and Louisiana Paternity Laws

Louisiana paternity laws govern the last name you give your newborn child and the recording of the newborn child's father on the birth certificate. The laws differ depending on whether the mother is married at the time of conception and birth. There are also laws governing the change of last name of a child and the recording of paternity after birth registration.

Fill Out the Birth Certificate

We recommend filling out your baby's birth certificate forms prior to delivery. Birth certificate packets are available when you preregister for delivery. Approximately three to six months after delivery, you’ll receive the birth certificate from the Louisiana Vital Records Registry. Learn more.
