How to Obtain a Copy of Your Medical Records During the COVID-19 Crisis
Due to the COVID-19 crisis we are encouraging patients not to come to the hospital for their medical records. However, you can still obtain copies of your medical records by following these directions:
- Complete this form indicating how you would like for us to process your request and return it to us by:

- Email: SMHMedRec@SlidellMemorial.org
If your doctor needs a copy of your records, please have them contact Medical Records at (985) 280-1706 or Fax: (985) 280-8897 and we can fax your records directly to their office.
