Birth Certificate Application Process

The Medical Records Department at SMH assists parents by completing newborn birth certificate applications.
To obtain a birth certificate application, please visit SlidellMemorial.org/Medical-Records. The application is located under “Birth Certificates.” Or click here to download. This needs to be filled out online and emailed directly to our Birth Certificate Clerk at: BirthCertificate@SlidellMemorial.org.
Our birth certificate clerk will visit you during your stay to sign the birth certificate application. The clerk is available during the following extended hours to assist you:
- Monday – Friday: 8 a.m. to 4:30 p.m. and 5 p.m. to 6:30 p.m.
- Saturday: 8 a.m. to Noon

***It is very important that you complete the birth certificate application BEFORE arriving at the hospital for delivery. Failure to do so will result in delayed discharge after giving birth.***
